Licensed Practical Nurse (LPN) - Part Time - Home Health - Personal Care

Care Advantage Inc, Voted #1 in Home Healthcare the last 5 years is looking for a Licensed Practical Nurse (LPN) to join our Personal Care nursing team in a Part-Time capacity.

Our team provides 1 on 1 care in patients' homes with extensive services to provide our patients with the best possible care. This position promotes and supports the Company's core values of Compassion, a positive Attitude, Respect and Excellence.

Our Licensed Practical Nurses assist with supervisory visits and follow-up visits based off the plan of care for patients located within the Newport News, Chesapeake and Portsmouth area as well as some of the surrounding areas.

The Licensed Practical Nurse will effectively interact with patients, caregivers, family members and other interdisciplinary co-workers to provide nursing care to patients on an intermittent basis.

A DAY IN THE LIFE OF A Licensed Practical Nurse

• Makes initial contact with the patients and explains services offered
• Completes follow-up visits as needed
• Instructs assigned care team members on the needs and care plan details
• Reports progress of care to involved parties
• Follows up and maintains relationships with clients, always advocating on their behalf
• Establishes relationships with community partners and referral sources
• Keeps Clinical Leadership and Office Administrator abreast of patient condition and/or medical changes
• Reviews caregiver documentation
• Takes part in interdisciplinary team meetings to ensure needs of all patients are met

Required Qualifications:

• Graduate of an approved school of professional nursing
• Current and valid Virginia LPN License
• Two (2) years' Personal Care home health nursing experience, preferred.
• Previous experience completing Supervisory visits, strongly preferred.
• Passionate and motivated to provide care within a patient's home.
• Ability to exercise initiative and independent judgment.
• Comfortable working collaboratively in cross-functional teams
• Ability to communicate effectively, both verbally and in writing, with all levels of an organization
• Ability and willingness to travel within a given territory.
• Willingness to travel to multiple patients in varying home locations.
