Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving addicted men, women, and Veterans through the provision of high quality, successful, and affordable residential and outpatient recovery programs.

Job Title:       Licensed Practical Nurse - DETOX

Reports To:   Director of Nursing

 FLSA Status: Non-Exempt (Hourly)

Position Summary: The Licensed Practical Nurse (LPN) is responsible for assisting in the individualized patient care by promoting and restoring clients' health through the nursing process; collaborating with interdisciplinary team members; providing physical and psychological support to clients, friends, and families.

 

Scope of Practice: Assessment of the physical needs of the client and implementation of a plan of care through preventative, and therapeutic services in accordance with all state, federal, and licensing standards and guidelines.

 

Minimum Qualifications:  

  1. Graduate of an accredited school of nursing;
  2. Current BLS certification; and
  3. Current and Valid State of Arizona Driver’s License Required.
  4. Current and Valid Finger Print Clearance Card.

Essential Duties and Responsibilities:

Direct Care

  • Acts in accordance within the scope of practice of an LPN
  • Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs.
  • Conducts ongoing assessments as determined by patient’s condition and/or the client facility’s policies, procedures or protocols and reprioritizes care accordingly.
  • Contributes to the plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare team.
  • Performs appropriate treatments as ordered by provider in an accurate and timely manner.
  • Performs therapeutic nursing interventions as established by individualized plan of care for the patient and his/her family.
  • Delivers care with appropriate age and cultural competence to specific patient populations according to individualized needs.
  • Provides individualized patient/family education customized to the patient and his/her family with RN assistance.
  • Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
  • Initiates emergency resuscitative measures according to adult resuscitation and advanced life support protocols.
  • Maintains confidentiality in matters related to patient, family and client facility staff.
  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity and rights.
  • Reports patient condition to appropriate personnel during each shift.
  • Maintains current competency in nursing specialty by attending educational workshops.
  • Orient the clients to the Clinic system and available services.
  • Act as an advocate in directing clients to available Clinic and community resources.
  • Educate clients regarding condition, treatment, and possible outcomes according to Patient Plan of Care
  • Collaborate with other members of the staff, faculty and with volunteers.

 

 

Administrative

  • Maintain accurate medical records using the EMR system and proper forms in a timely manner. Supervise and train clinical staff and volunteers on data entry into the electronic health records system and provide oversight that original data is entered correctly.
  • Assists in precepting volunteers.
  • Consistently use supplies and equipment in a cost-effective manner.
  • Recommend needed supplies and equipment for delivery of care.

 

Quality Assurance

  • Participate in quality improvement activities as directed by the Director of Nursing.
  • Maintain the assigned caseload to meet productivity needs.
  • Utilize updated written protocols as mandated by the Arizona Board of Nursing Statutes and Regulations.
  • Follow established safety precautions in performance of duties to ensure a safe environment.
  • Support policies and philosophy in delivering health care as set forth by Crossroads 360 Community Care Clinic and Crossroads Board of Directors.

 

Other Duties: Abides by all Crossroads Policies and Procedures.

 

Qualifications:

  • 1+ years’ clinical experience in a clinical care setting
  • Experience with clinical risk management and safety protocols
  • Willingness to adhere to all in-house and legal regulations
  • Familiarity with EHR and EMR records systems
  • Excellent communication, problem solving and organizational skills
  • Current CPR/First Aid training or obtained during Crossroads orientation.
  • Ability to work independently and as a member of a team.

  • While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to finger, handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.
