We are seeking a qualified Licensed Practical Nurse (LPN) with experience in medical record abstraction to join our team. In this role, you will be responsible for reviewing patient medical records and extracting key clinical data to support various healthcare initiatives, such as quality improvement programs, research studies, and regulatory reporting.

Responsibilities:

  • Accurately abstract relevant clinical data from patient medical records (electronic and paper) according to established protocols and guidelines.
  • Apply strong understanding of medical terminology, anatomy, and disease processes to interpret medical documentation.
  • Identify and code diagnoses, procedures, medications, and other relevant data points using standardized coding systems (e.g., ICD-10-CM, CPT).
  • Ensure data accuracy and completeness through meticulous review and adherence to quality standards.
  • Meet established deadlines for data abstraction tasks.
  • Maintain confidentiality of patient information according to HIPAA regulations.
  • May participate in ongoing training to stay updated on data abstraction protocols, coding systems, and relevant healthcare regulations.

Qualifications:

  • Current and active Licensed Practical Nurse (LPN) license in the state of employment.
  • Minimum of 1-2 years of experience in a clinical setting.
  • Proven experience with navigating medical charts, research, or regulatory reporting is highly desirable.
  • Strong understanding of medical terminology, anatomy, and disease processes.
  • Proficiency in electronic health record (EHR) systems and data entry skills.
  • Excellent attention to detail and accuracy.
  • Strong analytical and problem-solving skills.
  • Excellent communication and interpersonal skills.
