Summary

The Nurse/LPN at Envision Unlimited plays a crucial role in assessing and providing for the medical, health, and safety needs of clients in the assigned community living program(s). Under the guidance of a licensed registered nurse, the Nurse/LPN conducts client assessments, documents medical needs and interventions, monitors medication regimes, and communicates with program directors, medical personnel, and others as necessary. This role is essential in ensuring the well-being and quality of care for our clients. Essential Responsibilities

  • Perform onsite client assessments and training, as required.
  • Document specific client medical needs and interventions, including client progress notes as needed.
  • Monitor aspects of client medication regimes, review medication records, orders, and bubble packs, and review MARs.
  • Review client medical records on a monthly basis or more frequently if necessary.
  • Develop interventions designed to allow staff to meet the specific needs of the clients.
  • Communicate regularly with Program Directors, CLS Administrators, Division Director, Client’s Physicians, and other medical personnel.
  • Attend client staffing's and present nursing notes as requested.
  • Conduct staff training and skills observations on an ongoing basis.
  • Assist in the facilitation of the client’s hospitalization via admission, discharge/after care planning with visits to the hospital as requested.

Other Responsibilities

  • Provide staff development and training in the areas of health and safety and other related medical subjects.
  • Attend and participate in Agency and other in-service programs.
  • Participate on Division and/or Agency-wide committees, as assigned.
  • Perform other related duties as assigned.

SUPERVISORY RESPONSIBILITIES

No supervisory responsibilities.

Requirements:

Assesses and provides for the medical, health, and safety needs of the clients in the assigned community living program(s), under the guidance of a licensed registered nurse.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.


EDUCATION, SKILLS, and/or EXPERIENCE

  • Must have a current license from the State of Illinois as a Licensed Practical Nurse.
  • CPR Certification.
  • Valid driver's license and access to a reliable vehicle.
  • Two years of clinical nursing experience, preferably with 1 year working with the developmental disabilities population.
