Position Overview:

Capabilix is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in delivering patient-centered care and ensuring the well-being of our clients. You will work closely with healthcare professionals and other team members to provide clinical support, administer medications, and assist with various medical procedures.

Key Responsibilities:

  • Provide direct patient care under the supervision of registered nurses (RNs) or physicians, following established care plans and protocols.
  • Administer medications and treatments as prescribed by healthcare providers, ensuring accuracy and adherence to dosage and administration guidelines.
  • Perform routine nursing procedures such as wound care, catheterization, and vital signs monitoring with precision and attention to detail.
  • Assist with the assessment, planning, implementation, and evaluation of patient care, collaborating with interdisciplinary team members as needed.
  • Maintain accurate and up-to-date medical records, documenting patient assessments, interventions, and outcomes in electronic health record (EHR) systems.
  • Educate patients and their families about their health conditions, treatment options, and self-care techniques to promote wellness and disease prevention.
  • Monitor patient progress and report any changes in condition or concerns to the supervising RN or physician promptly.
  • Adhere to infection control protocols, safety standards, and regulatory requirements to ensure a safe and sanitary healthcare environment.
  • Participate in ongoing training and professional development activities to enhance clinical skills and stay current with best practices in nursing care.

Qualifications:

  • Graduation from an accredited practical nursing program and current licensure as a Licensed Practical Nurse (LPN) in the state of [insert state].
