Licensed Practical Nurse ( LPN)

LPN’s provide a large portion of direct patient care. LPN’s may be assisted by nurses’ aides (CNAs) and other assistants in some of their duties. LPNs are directed by doctors and nurses (RNs & nurse managers).

Qualifications:

• Graduate of an accredited school of nursing, or proof of successful challenge of LPN Exam.
• Minimum of one (1) year recent experience in a home care setting within the period of two (2) years.
• Current unrestricted LPN license in the State of Maryland
• Current CPR

Job Duties:
1. Assisting in assessing, planning, implementing and evaluating patient care;
2. Conducting and documenting ongoing data collection related to patient/family reporting to the RN following guidelines, policies and procedures;
3. Providing care to age specific patient population such as adolescent, adult or older adult-utilizing the nursing process;
4. Providing direct or indirect patient care services including but not limited to medication administration;
5. Demonstrating the ability to perform and respond appropriately in emergency situations;
6. Completing nursing care documentation for assigned patients;
7. Developing and maintaining effective communication with patient, families, physicians, colleagues and co-workers, to ensure the on-going provision of patient care;
8. Performing tasks and/or interacting with patients, families, staff and other personnel in a courteous, honest and compassionate manner.
9. Observe patients, charting and reporting changes in patients’ conditions, such as adverse reactions to medication or treatment, and taking an necessary action.
10. Administer prescribed medications
11. Answer patients calls and determine how to assist them
12. Measure and record patients’ vital signs, such as height, weight, temperature, blood pressure, pulse and respiration.
13. Help patients with bathing, dressing, and personal hygiene, moving bin bed, and standing and walking.
14. Work as a part of a health care team to assess patient needs, plan and modify care and implement interventions.
15. Record food and fluid intake and output
16. Evaluate nursing intervention outcomes, conferring with other health-care team members as necessary.
